The Sections
Sections have been created for data entry for the family to have separate areas for data entry rather than a great big long form of data and for clarity.
Each section may be hidden from the family if required.
You may not wish the family to fill in the Application for Cremation section. If this is the case – just hide it. see Help to Display to the Family.
Note: The Application for Cremation Questions section will only show for cremation cases (if not hidden).
The following lists the sections available and a description of what they contain.
Each section will replace the word deceased with the first name of the actual deceased or the nick name of the deceased (if entered by yourself or the family).
There is no screen grabs below as you will be able to just create a portal and look at the family’s view yourself while getting used to this feature.
In fact this is HIGHLY RECOMMENDED before going live with families.
Each section may adjust/change depending on the country/state the company is situated in.
Deceased’s General Details
This contains…
- The Title
- The Nickname (in case the family want to enter one)
- Middle Name/s
- Any different names at birth
- The residence
- The Occupation
- and the questions concerning being a marriage celebrant, Justice of the Peace and any honours
Deceased’s Date and Place of Birth
This contains…
- Date of Birth
- City/Town of Birth
- If not born in this country then there is the ability to a specify the country and to answer any other questions relating to this
- Any ethnicity questions
Deceased’s Mother
This contains…
- The option to change from Mother to Father or Parent
- The names
- The names at birth
- the occupation
Deceased’s Father
This contains…
- The option to change from Father to Mother or Parent
- The names
- The names at birth
- the occupation
Deceased’s Relationships
This allows the family to add in the relationships of the deceased.
We have made this as clear as we can with the relationships labeled as to their order.
This is one of the sections that may require some help for the family.
Deceased’s Children
In this section we only ask the information for the children that is required by BDM.
First we just ask for the first name and then the family can click ‘Add Son’ or ‘Add Daughter’. This means that we can set the sex of the child for them.
If they add a son – we set the surname to be the same as the deceased’s.
If they add a daughter we leave the surname empty for them to fill in.
As BDM need to know if any child is deceased or not we have a field for ‘This Child is.. either Alive (default) or deceased.
The age of the child is able to be entered here.
Any children added here will also be displayed in the next family members section.
The reason for this is that it is the children that often arrange the funeral and in the family section there is the ability for the contact phone and email to be added.
Additional Family (contact details)
This Section allows the family to add in any other family members they wish to.
The data they can enter is…
- Title
- Firstname/s
- Surname
- Relationship
- Gender
- Mobile Number
- Email Address
If the Family member entered’s relationship is not Son or Daughter then the age and status do not show.
Why is there no address?
Well – remember this is the very first interaction with the family.
Possibly before we have even met them in person.
As we do not really require the address – then why ask for it.
If you were using the portal, would you be comfortable entering this information?
Deceased’s Next of Kin
This contains the normal next of kin details.
Name address, relationship, phone numbers and email.
If you usually have this information in FreeDAM when you create the portal then consider hiding this section.
To Whom should the account be sent?
This contains the normal account details.
Name address, relationship, phone numbers and email.
If the next of kin has been filled in then there is a button that will appear that says…
‘Set this to…’ (adds the name of the next of kin to the button.
Application for Creamation Questions
This section WILL NOT display if the case is not for cremation.
This is a section that also might require a little bit of help written for it.
By it’s very nature this section is quite long.
All the questions are there for the family to answer along with the name and details of the applicant that can be set to the next of kin at a push of a button.
If the family have filled this out and you have accepted the data into FreeDAM before going to the arrangement – then it would be possible to print out a form a document all filled in ready for signing.
Terms and Conditions
The terms and conditions do not have help.
This is because you can just set these to be what ever you wish them to be in the portal settings.
Your terms and conditions will show in this section followed by the question specified in your Portal Settings and the answers (if you wish them to)
